In order to start the journey of therapy with us-click on the ‘forms’ tab and print out and the case history form.   Please return the form by fax (407-910-2923) or by email to billing@speechbuilders.org. If you have a referral from your Pediatrician- please fax it to us or email it to billing@speechbuilders.org.  If you are paying out of pocket then no referral is necessary.   You will receive an email once we input the information into our system and our office will call you to schedule an appointment.  Next we evaluate your child either in our clinic or at their school. If you have an evaluation that has been completed by another therapist in the last six months then we can use it. If not, an evaluation will need to be completed before we start therapy.

Speech builders, LLC currently accepts the following insurance plans:   Aetna (commerial), Aetna Better Health, CMS, Humana, *Simply, Sunshine State (under age 3),  United, TriCare, GEHA, FES-US (formerly Gardiner).  We are out of network for the following plans: BCBS, Cigna, and Humana. 

*At this time we are placing patients with Simply Insurance on our waiting list.  We update insurance information every month.
